Clinic Changes Effective 12/31/2020

We are transitioning Bellingham Health from a full Primary Care practice to a specialty Functional Medicine and Women’s Health practice effective December 31, 2020.  We will no longer be managing certain medications or providing 24/7 call.  This means that our patients will need a separate Primary Care Provider for their urgent and primary care needs.  We are posting this notice to help our patients understand and prepare for the change.

Telehealth Visits Available

During the COVID-19 outbreak, Bellingham Health is seeing as many patients as practical with telehealth video visits. This technology uses your computer or smartphone to connect you with your provider from the comfort of your home. This saves time and potential virus exposure for both staff and patients.
While telehealth is safe and convenient, it is not appropriate for all types of exams and health issues.  Please call our office to see if it is appropriate for your health needs.

Patient Information Forms

New Patients:  The two forms below are required for all patients.  Please print them, fill them out, and bring them to your first visit. Completing these forms in advance lets you to carefully consider the questions, provide complete answers, and it saves valuable time during the appointment. You may also bring any supplemental information that you feel is important.

Established Patients: Please use these forms to let us know if there are any changes to your personal, insurance, or health information. 

General Information and Health History

This form provides the basic health history and demographic information that we need to create your medical record and begin providing services. Please take the time to answer the questions completely.

Notice and Acknowledgment of Privacy Practices

The first two pages of this document are a Notice of Privacy Practices. The Notice describes how our clinic may use or disclose your personal medical information. The last page of this document is a form to acknowledge receipt of the Notice of Privacy Practices. All patients must sign the acknowledgment form and return it to the clinic before we can see you.

Release of Medical Records

We secure your medical records per HIPAA regulations. Your explicit permission is required to release them. If you would like us to share them with another person, medical office, or business, please complete the release of records form below and return it to our office. (Depending on the size and purpose of the release, a duplication/processing fee may be charged.)
